We started preparing for this as soon as school ended in June 2013. We were also preparing laptops for the 11th and 12th graders. Here are the steps we followed:
Sort iPads / charger & cable envelopes into carts
Order cases for new iPads
Determine when to replace displays (how much damage) - let them keep minor damage (tape it)
Check with Apple about removing apps from configurator so they can be used for each student - We found that Apple did this for the LexingtonOne school district, but they would not do this for us, so we have 9th grade iPads with no school owned apps - students will be given redemption codes. Tenth grade iPads will still get paid apps from Configurator (iMovie, Pages, Keynote, Notability).

Unpack
Place in case
Put 30 in cart to set up
Return to large box for storage (in alphabetical order)
Old iPads
Clean display
Check that ‘CYHS’ is still visible on back of iPad; replace if necessary (new iPads were engraved)
Check case; replace if necessary
Erase all content and settings
Go through setup
All iPads
Connect to Configurator
Sync for Profiles etc.
Name with student’s name (LastName, FirstName) (old ones - Rename from CYHS iPad xxx to student’s name )
In Assign, check that device has AUP files / assign to student
Check name label - replace if necessary
Remove # label if present
Set up spreadsheet / mail merge (in Google Docs) for redemption codes for 9th Graders

'Fix' Disabled iPad (wrong passcode entered too many times)
Sometimes just Refreshing this with Configurator will work
Sometimes iPad needs to be Unsupervised and set up from the beginning.
Now that students update most apps (except 10th grade paid apps), it is much easier. It is still challenging to update Notability etc. Just doing this as needed.
Configurator 1.4 and OS X 10.8 are required for upgrading (as of 9/25/13 this is not working properly. More info:

New procedure for students turning in iPads (broken or student withdrawing from school)
Turn off passcode
Turn off Find My iPad
Wait until end of day or next day to erase iPad if needed
For withdrawn students:
Erase iPad
With Configurator, unsupervise iPad
Name TBD
For damaged devices:
With Configurator rename (remove student's name, keep Loaner name)
In Feb. 2014, Apple announced a program that allows organizations to enroll devices and manage them without IT staff touching the device - without using Configurator We could still use our Lightspeed MDM. We should look into this for the future. It also looks like we can enroll devices we already have, although I haven’t determined if they will allow us to re-use the vpp app codes for the class of 2016.
